Is this a complete cushion compact or only the refill?

This is the Re-Feel refill, not a complete compact. The compatible Don't Touch Glass Pink Cushion case is sold separately.

How do I install the refill?

Remove the cartridge from the right side of the compatible case, then insert the new cartridge until it clicks.

How do I dispense and apply the foundation?

Press the button on the right side of the case. Pick up the dispensed foundation with the puff and gently pat it over your face in an even layer. Add thin coats where you want more coverage.
